Here are the issues:

No need to put a citation before you put the exact same citation near the end. It looks like this: William Howard was a dentist.[10] He was born in 1980. [10]

I found one instance. It was in a paragraph where two significantly different concepts needed citation, and coincidentally were referenced by the same source, not necessarily on the same page.
